Psychology of Scars

Scars are more than just fibrous tissue; they are permanent changes that can define a person’s character and impact their entire quality of life. For the practitioner, treating a scar isn’t just about aesthetics—it’s about restoring a patient's social confidence and performance.

In this episode's highlights, get some of the key insights discussed, including:

  • Understand the data behind the Dermatology Life Quality Index (DLQI) and how scars contribute to social avoidance, sadness, and work performance issues.

  • Learn about the role of growth factor TGF-beta 3. Discover why traditional ablative treatments often fail long-term and how non-ablative microneedling promotes the specific growth factors required for healthy tissue.

  • Gain clinical clarity on mechanically breaking the collagen matrix

  • Review award-winning case studies and learn integrated protocols.

The role of connecting psychiatric insights and clinical microneedling protocols

We recently hosted a global webinar featuring consultant psychiatrist and aesthetic practitioner Dr Maciej Danilewicz, alongside our UK Country Director, Kelly Tobin. The session delved into the profound "psychology of scars," exploring how permanent textural changes can lead to significant psychological distress. As regenerative medicine evolves, Dr Danilewicz highlighted how evidence-based protocols that prioritise tissue reorganisation are essential for restoring patient confidence and quality of life.

The impact of scarring on quality of life

Research presented during the webinar revealed the startling reality of living with facial scars. A cross-sectional study of 723 adults with facial acne scars showed that over 75.4% of participants were deeply concerned about their appearance, with 76% feeling chronically self-conscious.

The psychological burden extends far beyond surface-level aesthetics:

  • Social and professional limitation: 48.1% of participants reported that scarring impacted social activities, while 41.7% felt it hindered their work performance.

  • Emotional distress: 68.4% of respondents associated their scarring with persistent sadness.

  • Significant disability: For 22.5% of patients, the impact on their quality of life was so extreme that it was classified as a form of disability.

Dr Danilewicz emphasised that even a minor improvement in the appearance of scars can positively impact a patient's relationships, social activities, and self-esteem.

  • To effectively diminish the appearance of scars, practitioners must understand the biological "reset" required during the wound healing process. Dr Danilewicz detailed how the Dermapen 4® delivers regulated biostimulation that goes beyond traditional ablative treatments.

  • Traditional ablative treatments like lasers often promote the production of Collagen III, which can lead to further fibrotic tissue. In contrast, non-ablative microneedling with the Dermapen 4® promotes the release of Transforming Growth Factor beta 3 (TGF-β3), which is associated with the formation of softer-looking, net-like Collagen I.

  • For deep, stubborn, or fibrotic scar tissue, precision is paramount. The Dermapen 4® features a dedicated Scar Treatment setting with autocalibration, allowing aesthetic professionals to safely reach depths of up to 3mm. This mechanical stimulation breaks down the parallel-arrayed Collagen III fibres, allowing for true tissue reorganisation.

  • A major innovation in clinical protocols is the integration of exosome technology. Using Dp Dermaceuticals EXO-SKIN™—which contains biomimetic synthetic exosomes—catalyses skin transformation. These cellular messengers carry growth factors that modulate inflammation and amplify the regenerative response when applied alongside microneedling.

    • Controlled cascade: The Dermapen 4® delivers 1,920 punctures per second, creating millions of micro-channels that initiate a controlled healing response without excessive trauma.

    • Non-negotiable home care: Successful outcomes depend on maintaining skin integrity post-treatment. Dr Danilewicz identifies Dp Dermaceutical RETINAL ACTIVE™ (Vitamin A) and DP Dermaceutical SSSCAR™ (silicone-based) as non-negotiable for enhancing scarless healing.

    • Managing expectations: Professionals must distinguish between understandable distress and conditions like Body Dysmorphic Disorder (BDD). A thorough consultation is crucial to ensure realistic expectations, as microneedling aims to diminish the appearance of scars rather than erase them entirely.
